Start with a blank form or a template
Google Forms is a free Google tool for making questionnaires, surveys, sign-up sheets, and feedback forms. You can build a form from scratch, choose the answer format for each question, send people a link, and review the responses as they arrive.
Open forms.google.com in your browser to get started. You can also open Google, select the nine-dot Google apps icon in the top-right corner, scroll to Forms, and select it.
The Forms home page includes templates for common jobs, including event registration and contact forms. A template gives you a useful starting layout. If you want to control every question yourself, select Blank form.

Create your Google Form

At the Google Forms home page, select Blank form, or select a template that matches what you need.

Click the form title and enter a clear name. Use a title that tells respondents what the form is for, such as Event Sign-Up or Customer Feedback.

Add a description below the title if respondents need instructions, a deadline, or other context before they begin.

Enter the text for your first question. Keep each question focused on one thing so people can answer it quickly and accurately.

Use the question type menu on the right side of the question to choose how people will respond. Google Forms includes options such as Multiple choice, Checkboxes, File upload, and Short answer.

Add the answer choices when you use a question type that needs them. For example, a multiple-choice question needs the options a respondent can select.

Turn on Required for any question that people must answer before they can submit the form.
Add and organize form content
The toolbar on the right lets you expand the form beyond its first question. Select the option to add another question whenever you need one. You can also import questions from another form instead of writing them again.
Use the same toolbar to add a title and description within the form, an image, or a video. These items are useful when a longer form needs instructions between groups of questions or when you want to give respondents extra context.
You can also add sections. Sections split a long form into smaller parts, which can make a registration form or survey easier to work through. If you add a section you no longer need, select the trash can at the bottom of that section to remove it.
For questions or other items you want to reuse, select the duplicate option. This is faster than recreating a similar item from the beginning. It is especially helpful when several questions use the same answer format.
Pick the right question type
Your question type controls the kind of answer you collect. Choose it based on what you need to learn, not just what looks best on the form.

Short answer works for brief written responses, such as a name, email address, or short comment.

Multiple choice works when a respondent should pick one answer from a list.

Checkboxes works when more than one answer can apply.

File upload works when you need respondents to provide a file as part of their response.
Before sharing the form, read every question as if you were the person filling it out. Check that each option is clear and that all important questions have Required enabled.
Review settings and responses
Select Settings in the form to configure the available options for your form. Review these settings before publishing, especially if you are collecting responses for a specific group or purpose.
Select Responses to view the answers people have submitted. As respondents complete the form, their answers appear there. This gives you one place to check response activity and review individual answers.
If you want to analyze the information in a spreadsheet, you can import the responses into Google Sheets. This is useful for sorting answers, comparing responses, or doing additional analysis after you collect them.
Publish and share the form link

When the questions and settings are ready, select Publish in the top-right corner of Google Forms.

After publishing, use the link shown at the top to share the form with your responders.

Select the short URL option if you want a shorter version of the link.

Copy the link and send it to the people who need to complete the form.
Respondents can open the link, answer the questions, and select Submit. Return to the Responses area in your form to see their answers. If you make changes later, check the form again before sharing the link with a new group of respondents.
